Last week, KHA expressed serious concern with certain aspects of the Centers for Medicare and Medicaid Services’ (CMS) proposal for implementing a site-neutral payment component to the Long-Term Care Hospital (LTCH) Prospective Payment System (PPS) for cost reporting periods beginning on or after October 1, 2015.

“KHA supports many of CMS’s proposals, such as the proposal to use a specific 15 Medicare-severity-LTC-diagnosis-related groups to identify the psychiatric and rehabilitation conditions that would be paid a site-neutral rate versus a standard LTCH PPS rate, and KHA supports the proposal to use the previously finalized pressure ulcer measure to meet IMPACT Act requirements,” wrote KHA Membership Services Director Pam Kirchem, commenting on LTCH provisions of the proposed inpatient and LTCH PPS rule for fiscal year 2016. “However, we believe that the proposed two outlier-related budget neutrality adjustments are unwarranted and result in inappropriately lower LTCH payments. Baptist Health Paducah and hospital Baptist Health Madisonville have both received federal approval to move forward with plans to develop long-term acute care hospitals (LTACHs) within each facility. Baptist Health is partnering with ContinueCARE Hospital, a not-for-profit Community Hospital Corporation subsidiary based in Plano, Texas, to operate the new hospitals within the current facilities at Paducah and Madisonville.

The first stage of the new “hospital within a hospital” program for long-term patients will both open at Baptist Health Madisonville and Baptist Health Paducah this summer, eventually saving 540 families a year in Madisonville and 800 families a year in the Paducah area from having to travel out of the area for care.

During the demonstration phase, the hospitals will meet the needs of their patients instead of transferring them out to a distant facility. Once licensed, the local facility will be able to accept more patients from neighboring hospitals.

Last week, KHA submitted comments to the Centers for Medicare and Medicaid Services (CMS) on the fiscal year (FY) 2015 proposed rule for long-term acute care hospital (LTACH) prospective payment systems (PPS).  The comments focused on:

Proposed changes to the greater than three-day interrupted stay policy; Elimination of the five percent readmission rule that pertains to interrupted stays in co-located LTACHs; Proposed changes to the long-term acute care quality reporting (LTACHQR) program; and FY 2018 Measurement Proposals. On Thursday, June 12, CMS released a notice regarding changes to the Low Volume Hospital (LVH) and Medicare Dependent Hospital (MDH) exception payments for the last 6 months of FY 2014.

This document addresses payment for these programs only for the second half of fiscal year (FY) 2014 (April 1, 2014, through September 30, 2014). CMS proposed to implement the statutory changes for the first half of FY 2015 (October 1, 2014, through March 31, 2015) in the FY 2015 Inpatient Prospective Payment System (IPPS)/Long-Term Care Hospital (LTCH) PPS proposed rule that appeared in the May 15, 2014 Federal Register.

The Healthcare Financial Management Association (HFMA) Kentucky Chapter Winter Education Institute will be held at the Hyatt Regency in Louisville on January 23-24. Attendees can earn 12.6 CPE credits and will learn from a panel of six Kentucky hospital chief executive officers about Long-Term Acute Care Hospitals (LTACHs) and how utilizing the LTACH level of care can improve patient outcomes and a community hospital’s bottom line. Attendees will learn how LTACHs are quality, cost-effective alternatives to long stays in a community hospital ICU, and the importance of managing average length of stay (ALOS) for patients who will move to post-acute care (PAC) providers. 

There are eight LTACHs in Kentucky. They are acute care hospitals, treating critically ill patients who are stable, but still very sick with complex medical conditions. LTACHs offer slower and more supervised care for the sickest of patients; about 2 percent of all Medicare patients are discharged to an LTACH where a team of doctors and nurses can continue to take care of the patient's complex needs. LTACHs are not nursing homes or skilled nursing facilities (SNFs).
